ResponsibilitiesFranciscan Medical Group is currently seeking a full-time RN-Clinic Cardio -DEVICE for our Franciscan Heart and Vascular in King County, WA. We are seeking a motivated individual to join our patient centered team. This individual will support a fast paced Cardiovascular care team. Day shift, no weekends, no organizationally recognized holidays required! 4 day work week.Job Summary:This job responsible for providing professional nursing care focused on the testing, retrieval and analysis of information stored in implantable cardiac devices (e.g. loop recorders, pacemakers, defibrillators, etc.) in an ambulatory care setting consistent with CHI Franciscan Health (CHI-FH) standards/values, Franciscan Medical Group (FMG) Scope of Practice policy and applicable regulatory requirements. Work is performed following established standards, protocols and practices, and within scope of RN licensure, including providing pre- and post-procedure counseling and education to patients/families with respect to arrhythmia interpretation and management of implanted cardiac devices.Work, as ordered by a physician, includes: 1) providing perioperative recommendations regarding programming of patient’s implanted device relative to scheduled procedures; 2) performing in-clinic interrogation and reprogramming of implantable loop recorders, pacemakers and automatic implantable cardiac defibrillators (AICD); 3) performing routine remote interrogation and monitoring for daily device alerts; 4) developing initial device analyses and generating preliminary reports; 5) triaging and conducting visits to patients post-device insertion; and 6) alerting practitioners to urgent issues requiring further action.Work is performed under standards of safety and care that provide instruction/guidance for taking care of issues and patient needs in the absence of a physician/provider. Work requires the application of judgment and critical thinking skills in making decisions regarding patient care (within the scope of practice), including knowledge and integration of available standards, protocols and data, and in the efficient utilization of staff/resources. Work involves continuous contact with physicians/providers, patients, patient families and multidisciplinary care teams in performing cardiac device activities. An incumbent is accountable for the quality of care provided to patients, and has the authority to direct care, provide education, and delegate appropriate tasks.Work requires the ability to apply professional nursing skills to evaluate patients with arrhythmias and the technical skills to evaluate patients with cardiac rhythm management devices as well as the ability to program settings and troubleshoot devices in collaboration with cardiology providers and device representatives. Work also requires the expertise to assess data from patients and cardiac equipment/tests to collaborate effectively with providers on appropriate treatment protocols/decisions relating to current and future health status. This position functions in a highly specialized and unique area which requires a high level of clinical acumen and independence.Essential Duties:Administers cardiovascular and pharmacological stress tests with the use of imaging modalities and IV therapy as appropriate; interprets EKG findings in accordance with standard protocols.

Completes cardiac risk assessment forms in response to requests from internal/external providers for cardiac clearance for new/additional clinical/diagnostic tests/protocols; forwards assessments to Cardiologist for final determination.

Reviews protocols established for each procedure/study and provides patient with specifically-tailored training/information based on unique patient medical history and physician-specific protocols.

As initial point of patient contact, performs nursing triage activities based on direct patient contact or via phone within scope of licensure; independently triages cardiac patients/situations in accordance with standing orders/protocols; addresses more complex, urgent or emergent situations escalated from other clinic staff.

Documents studies, procedures, and interactions in patient’s electronic medical record in accordance with established procedures; enters, pends and signs orders as appropriate.

Device:Provides technical, clinical and related services in the areas of Cardiology and Electrophysiology (EP) relative to Cardiac Implantable Electronic Devices (CIED); performs interrogations of implanted cardiac pacemakers, defibrillators and loop recorders in an ambulatory setting, or through remote transmission review, in accordance with established clinical protocols and guidelines.Performs interrogation, evaluation, and reprogramming of all manufacturers’ models of cardiac rhythm management devices; triages phone calls and completes programming orders according to established protocols; performs in-office device checks, applying considerable knowledge of device reprogramming steps per established protocols. Sets up, operates, and troubleshoot various CIED programmers and deciphers complex cardiac device data elements; provides Implantable Cardioverter Defibrillator (ICD), Pacemaker (PM), Cardiac Resynchronization Therapy Defibrillator (CRTD), Cardiac Resynchronization Therapy-Pacemaker (CRTP) checks in-clinic and via remote follow-up. Manages device data in all available applications and patient medical records; communicates all findings to include indications found during testing; training and educating patients on all aspects of the proper use of transmitting devices; troubleshoots patient compliance issues; schedules appointments for pacemaker monitoring and facilitates appropriate in-office follow up. Performs clinical review and interpretation of in-office or transmitted data and informs EP/Cardiology providers of any urgent device issues; identifies opportunities for process improvements as well as positive problem resolutions. Examines post-implant wound site and performs associated patient care, including suture removal for post-procedure and implanted device patients. Performs follow up testing of all cardiac rhythm management devices, including remote monitoring under the direction of the physician.

Manages and coordinates pre- and post-procedure patient education, scheduling and procedural nursing follow-up for all implantable device procedures. Reviews protocols established for each procedure/study and provides patient with specifically-tailored training/information based on unique patient medical history and physician-specific protocols. Educates patients and family members on device-related topics and remote transmitters; communicates various cardiac device applications to patients in a coherent manner. Coordinates and provides pre- and post-operative instructions on multiple different device implantations and LAAC procedure as well as evaluations for wound infections post-procedure. Provides post-procedure instructions to patient regarding what to do and who/when to call with questions or concerns.Performs cardiac device-related nursing triage activities based on direct patient contact or via phone within scope of licensure; independently triages cardiac patients/situations in accordance with standing orders/protocols; addresses more complex, urgent or emergent situations escalated from other clinic staff. Applies professional clinical judgment in assessing complex patient problems/needs in accordance with established protocols; identifies potentially emergent or critical issues which require immediate action or interventions; determines whether patient acuity/symptoms warrant a clinic visit and/or immediate referral to the ER or physician/provider. Executes nursing interventions and/or plan of care to facilitate the patient’s achievement in acquiring their highest level of function; visits patients who are post-device insertion to monitor status/progress. In collaboration with physician providers, reviews and assesses data from tools/equipment/devices (e.g. ECG, Holter Monitors, lab studies, remote device interrogation, etc.) relating to patient current health status and makes appropriate treatment decisions; applies professional clinical judgment in making recommendations Provides guidance and mentoring for peers on device-specific protocols and technologies.Documents studies, procedures, and interactions in patient’s electronic medical record in accordance with established procedures; enters, pends and signs orders as appropriate. Reviews remote and in-office evaluations and completes documentation according to protocols. Acts as backup to enroll patients into approved research studies as instructed by principal investigating physician and follows study reporting and monitoring requirements. Takes verbal orders from a provider (in person or over the phone) and ensures orders are signed off in accordance with established procedures.Cultural Sensitivity and Competence: Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Population Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.Performs related duties as required.QualificationsEducation/Experience:Graduation from an accredited school of nursing approved by the Washington State Board of Nursing and two years of related work experience that would demonstrate attainment of the requisite job knowledge/abilities. Work experience in a cardiology-related field is preferred.

Work experience in med/surg, telemetry and/or a cardiology-related field is preferred.

License/Certification:Current licensure as a Registered Nurse by the Washington State Board of Nursing.

Current Advanced Cardiac Life Support (ACLS) certification.
