As a Licensed Practical Nurse (LPN) supporting referral management services, you’ll review and coordinate specialty care referrals, educate patients about next steps, and collaborate with clinical teams to promote timely access to appropriate care.You’ll work across electronic healthcare systems to manage referrals from initiation through closure, maintain accurate documentation, track clinical results, and identify patients who may benefit from additional care coordination.Employment Type: Full-timeCompensation and Benefits:$24.30–$27.00 per hourUp to $5.09 per hour in Health and Welfare benefits, in accordance with the SCA80 hours of paid time off56 hours of sick leave11 paid holidaysSchedule: Monday–Friday, 7:30 a.m.–4:30 p.m. The position does not work federal holidays or designated facility closure days.Key ResponsibilitiesReview specialty care referrals for clinical appropriateness, administrative completeness, covered benefits, and required tests or preparation.Coordinate referrals and appointments with patients, referring providers, specialty clinics, and other healthcare partners.Educate patients about referral procedures, laboratory requirements, specialty appointments, diagnoses, and available healthcare benefits.Initiate, monitor, and close referrals within established timelines while maintaining accurate documentation in applicable healthcare systems.Track consultation results, incorporate records into the appropriate medical record, and return results to referring providers.Identify patients who may benefit from case management or alternative levels of care and coordinate with the appropriate clinical team.Collect and analyze referral data, support quality and process-improvement activities, and provide oversight of referral management staff and processes as directed by the onsite quality representative.
