Summary:
Under the general supervision of the Rehabilitation Services Manager, the Case Manager I provides outreach and case finding, case management, assessment and planning, and case coordination between client, family and community resources. Provides monitoring and advocacy for individuals who suffer from severe and persistent mental illness. Works to develop and facilitate rehabilitation activities that include skills development, and recreational opportunities.

Maintains regular contact with outside resources and providers. Advocates on behalf of the consumer to assist qualifying clients/families to identify, obtain and secure needed employment, entitlements, supports, and services.
Maintains accurate client service records and distributes completed information to service providers and medical records within the established time frame.
Review treatment plans, monitor progress, and provide necessary follow-ups as required by the current Division of Behavioral Health regulations.
Provides and participates in interdisciplinary quality assurance, utilization review, treatment planning, treatment assessments and function assessment.
Monitors and coordinates client services and the service delivery system to ensure the care is appropriate, medically necessary, clinically justifiable, and active treatment.
Provides direct services to include individual and group skills development and crisis intervention. Researches and develops curriculum and prepares class materials to teach skills building classes.
Develops competencies through participation in supervision, case managers’ meetings, and required training in the areas of client engagement, documentation, caseload management and identifying and obtaining resources/entitlements for qualifying clients.
Reaches 70% of case manager productivity standard by completion of year 1 in the position and 100% of productivity standard by completion of year 2.
