Participate in community planning related to the interests of the facility and the services and needs of the residents. As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Perform administrative duties such as completing medical forms, reports, evaluations, and charting as required. including protected health information.Reports to: Director of NursingFLSA: Non-exempt Minimum Qualifications:Minimum Education

x High School Diploma required

x Associate’s degree preferredX CNA in the State of AZ preferred
