Overview :As a Care Navigator, you'll play a pivotal role in ensuring our members and their families have a warm and positive experience. From guiding members through seamless check-ins to meeting their care needs, you'll be committed to delivering exceptional service. We're looking for someone with strong customer-service skills, a genuine passion for senior care, and a proactive approach to ensuring every member's experience is exceptional.Responsibilities:
Welcome members warmly upon arrival, creating a safe and comfortable environment.
Maintain and update member information in our electronic medical records system.
Coordinate appointments and referrals across a diverse care team.
Assist members with electronic paperwork using our user-friendly kiosks.
Manage medical records and ensure accurate documentation in our electronic systems.
Address inquiries from prospective and current members and their families.
Efficiently handle incoming calls, taking detailed messages and ensuring information reaches the right individuals.
Support center events as needed.
Skills and Abilities:
Exceptional customer service skills with a positive and welcoming demeanor.
Dedication to providing high-quality experiences for our senior members.
Ability to collaborate effectively with diverse populations and stakeholders.
Familiarity with medical terminology, insurance processes, and electronic medical record systems.
Strong organizational skills with attention to detail.
Clear written and verbal communication abilities.
Proficiency in Microsoft Office Suite and general PC skills.
Adaptability and professionalism in a dynamic work environment.
Education and Experience:
Preferred: Associates degree or equivalent experience.
Minimum one year of clinical or similar work experience.
Experience with electronic medical record systems is advantageous.
