The Program Coordinator serves as a resource to staff the inter-disciplinary team and community caring for the patients within specific area. Demonstrates knowledge in the care of patients with specific diagnosis.
Coordinates supplemental treatment, admission/discharge and follow-up appointments for patients.
Independently acts as concierge for all patients which includes coordination of benefits, hospital stay coverage, appointments and hotel accommodations if needed, provides leadership in coordination of care.
Coordinate Consensus Committee Meetings, identifying patients ready for presentation, prepare clinical information, imaging, send meeting invites to physician team.
Act as a liaison for all patients and their families.
Provides support and information to physicians, residents and families regarding patient's needs and actively participates in process improvement.
Develops brochures and marketing materials for patients, families, staff and outside physicians.
Participates in community out-reach by meeting with community physicians, provide information about program, and establish a line of communication between BNI and physicians in the community.
Independently maintains patient data base for neurology team.
