The Home Health Assessment Registered Nurse is responsible for providing and documenting skilled one-on-one nursing assessments in accordance with the physician-ordered individualized patient care plan. The Assessment RN will be conducting in home assessments for members Monday – Friday, 8a-5p (40 hours). Must have dependable transportation as this is a road warrior type of position. The members do not require medical treatment. If they are recommended to do so from assessments, they are referred to the Program Clinic, or other medical facilities. Essential Duties and Responsibilities:

Completes an initial assessment of patient and family to determine healthcare needs when applicable

Provides a complete physical assessment and history of current and previous illness(s)

Regularly re-evaluates patient nursing needs

Initiates the plan of care when directed and makes necessary revisions as patient status changes

Uses health assessment data to determine nursing diagnosis

Establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions

Includes the patient and the family in the planning and execution of the treatment plan and goals

Initiates appropriate preventative and rehabilitative nursing procedures

Provides healthcare instructions and education to the patient as appropriate per assessment and plan of care

Identifies discharge planning needs when applicable

Adheres to home health agency policy and protocol

Attends team meetings, and education in-services as required

Minimum Requirements:

Graduate of an accredited school of nursing

Currently licensed as a Registered Nurse in the state in which the RN will practice

Complies with all relevant professional standards of practice

Participation and completion of Amergis' Competency program when applicable

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable to job site

Understand patient confidentiality and HIPAA requirements

Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

Computer proficiency required

Must be at least 18 years of age
