Care Manager

Senior Concerns in Thousand Oaks, CA is hiring a Care Manager to run the Caregiver Resource Center. The ideal candidate has a masters in social work or related field, and has experience working with older adults and family caregivers.

This Care Manager will:

Provide Case Consultations for Family Caregivers

Facilitate Caregiver Support Groups

Manage caregiver grants

Provide information and referral to older adults and family caregivers

This is a full time position, Monday through Friday, located at Senior Concerns in Thousand Oaks
