Description We are offering a short term contract employment opportunity for a Case Manager in San Francisco, California. This role involves managing a caseload of individuals who were previously homeless, many of whom have mental health disabilities, substance abuse issues, and/or medical issues. You will conduct outreach visits, perform needs assessments, and provide a full range of case management services.Responsibilities: Manage a diverse caseload of individuals, regularly conducting outreach visits and assessments. Provide comprehensive case management services, focusing on housing retention and quality of life improvement. Support individuals in maintaining their housing through rapport building, frequent engagement, and effective intervention strategies. Assist individuals with maintaining appropriate benefits and making rent payments. Address unit habitability issues and accompany hotel managers on inspection visits. Refer individuals to job placement programs as needed, based on their employability. Foster an active community environment by hosting regular community events and social activities. Refer individuals to appropriate services based on their needs and support successful connection to these services. Participate in agency-wide initiatives such as annual low-cost energy program recertification or annual income recertification. Maintain thorough documentation and database records of individual contact and services provided. Respond to tenant-related crises, providing de-escalation intervention and support as required. Collaborate with other departments and outside providers to support and advocate for individuals. Prior experience in a case management role is preferred. Excellent administrative skills, including proficiency in Microsoft Office Suite. Demonstrated experience in data entry and maintaining accurate records. Familiarity with various types of filing systems and effective file management practices. Strong interpersonal and communication skills to interact with diverse groups of individuals. Ability to maintain confidentiality and adhere to ethical standards. Excellent organizational skills and the ability to manage multiple tasks effectively. Demonstrated ability to work independently and as part of a team. Strong problem-solving skills and the ability to make sound decisions. Familiarity with local community resources and services is desirable. Ability to work flexibly, including evenings and weekends if required.
