Description We are offering a short term contract employment opportunity for a Case Manager based in San Francisco, California. You will be part of a team that assists individuals who were formerly homeless, many of whom may have mental health disabilities, substance abuse issues and/or medical issues. The role involves managing a caseload, conducting outreach visits, and providing a full range of case management services with the goal of housing retention and life quality improvement.Responsibilities: Manage a caseload of individuals, providing necessary support and intervention strategies Conduct initial and ongoing needs assessments to identify the requirements of the individuals Assist clients in maintaining their housing, through engagement and relationship-building Aid in establishing and maintaining appropriate benefits for clients and ensuring regular rent payments Handle unit habitability issues and accompany hotel managers on monthly inspections Refer clients to relevant job placement programs as needed Collaborate with other Case Managers and Property Management staff to build an active community by hosting regular events and social activities Facilitate connections between clients and appropriate services based on their needs Participate in agency-wide initiatives such as annual recertifications Provide support to hotel staff and clients on monthly check disbursement day Maintain accurate documentation and database records of client contact and services provided Respond to client-related crises and provide de-escalation intervention when necessary Collaborate with various departments and outside providers to advocate for clients Maintain confidentiality of client records and concerns Respond to all phone messages, client requests, staff requests, and emails in a timely manner.
