Groundbreaking cardiac procedures at Dominican include the Transcatheter Aortic Valve Replacement (TAVR) procedure which allows for heart valve replacement without opening a patient’s chest.Dominican Hospital has forged significant partnerships with area hospitals to bring exceptional care to Santa Cruz County. The hospital offers a level III Neonatal Intensive Care Unit (NICU) through a partnership with Lucile Packard Children’s Hospital Stanford. Neurosurgery at Dominican is also offered through a partnership with Stanford Health Care.ResponsibilitiesThis position is responsible for providing administrative support for Dominican Hospital's Community Health Integration Services and services that are directed toward the provision of a broad range of health services for the uninsured, underinsured and vulnerable populations of Santa Cruz County. Said programs may include: The Mobile Wellness Clinic, Every Woman Counts/Early Breast Cancer Detection Program, Women Helping Women Program, the RotaCare clinic, and the hospital discharge follow-up calls. In addition, the employee will provide coverage for the Community Health Coordinator on the Wellness Unit , complete initial patient assessment, NP/PA support, and discharge of patients as needed. This position will also be responsible for supply inventory and stocking of supplies on the Wellness clinic, and will provide and maintain Point-of-Care testing and immunization logs.QualificationsRequired:

At least two years experience providing health education in a community or public health clinic under the supervision of medical staff

Medical Assistant Certification

Current BLS certification

Current and valid CA Drivers License

Knowledge of community-based health resources

Excellent oral and written communication skills

Proven cultural competency in target client population

Preferred:

Bachelors Degree

Community Educator Certificate

Bilingual English/Spanish
