Members of the MemorialCare Epic Population Health job family, work side by side with our end users to install the Epic software, help lead and manage workflow/software changes, and ultimately transform the way that MemorialCare provides healthcare. MemorialCare's Epic Team develops creative strategies to achieve common end goals while collaborating with our end users. The Epic Specialist Healthy Planet will drive collaboration across MemorialCare to ensure that Epic and its integrations are designed for interoperability, per defined policies, as well as industry best practices. With their recognized mastery knowledge of Population Health, they will positively influence change for end users that maximizes the value of the system. The Epic Specialist Healthy Planet is responsible for developing and implementing an enterprise data strategy that aligns information assets with business and technology strategy. They apply in-depth knowledge of EpicCare and Healthy Planet applications to ensure effective and optimal use of systems. It is essential for this individual to be highly collaborative as they need to influence senior business leaders, functional leadership, as well as project and application managers.

Essential Functions and Responsibilities of the Job
  • Apply in-depth knowledge of Healthy Planet and EpicCare applications to ensure effective and optimal use of Epic providing recommendations on how the system can remain efficient and economical.
  • Stay aware of current and emerging data technologies and trends, and Epic changing functionality to provide advice to MemorialCare based on these modifications.
  • Lead internal projects providing regular reports of status and escalating as necessary.
  • Maximize the maintainability of Epic, keeping customization at a minimum.
  • Provide high-level customer service; accountable for end-user satisfaction and the success of assigned projects.
  • Act as user liaison between professional departments and Information Systems.

  • Experience
    • Minimum of 5 years experience in relevant Epic modules or similar experience required.
    • Minimum of 3 years of population management, case/care management, quality, accountable care organization (ACO), and/or patient portal required.
    • Has held a previous role formally or informally leading projects or teams.
    • Progressive project management experience and demonstrated effective communication working independently as well as in a team situation meeting aggressive deadlines and established goals.
    • Experience leading projects requiring an end-to-end EpicCare and Healthy Planet workflow perspective, a high-level overall understanding of the application, and considerable knowledge of EpicCare and Healthy Planet end user processes, effectively dedicating time across more than one project, and meeting established deadlines.
    • Demonstrated use of relevant out of the box thinking in conjunction with organization wide standardization.
    • Experienced working in a fast-paced environment with minimal supervision and managing multiple priorities.
    • Demonstrated ability to effectively utilize system tools to meet functionality needs of users including proficiency with information systems technology such as Microsoft Office Products required.
    • Experienced in leading projects, providing expert consultative guidance and direction on change initiatives.
    • Proven ability in workflow and data analysis and design techniques, including needs assessment, specification development, quantitative methods, planning and gap analysis.
    • Past success leading meetings between business stakeholders, technical resources, and third parties for business requirements and technical solutions.
    • Experienced working with a diverse, multi-disciplinary team, and interacting with all levels of the organization.
    • Demonstrated working knowledge in the following: Epic Radar; Epic SmartForm configuration; analysis and development of case management and/or other population health related workflows and their roles in data capture.
    • Demonstrated experience in Population Health Epic tools such as Risk Scores, Predictive Analytics, Integration of external data in Healthy Planet tools and Healthy Planet Link preferred.

    Education
    • Bachelor's degree or an equivalent combination of education and relevant experience required.
    • Established knowledge in EpicCare and Healthy Planet is required; certification required prior to or within first year in new role. Knowledge of other relevant Epic modules is preferred.
