Job description:

Under the direct supervision of the QIDP/Administrator, the Facility Manager (FM) is responsible for the management of a residential facility for the care and supervision of Intermediate Care Facilities (ICF) clients. Peppermint Ridge serves people with intellectual disabilities in accordance with mandates of public law, state regulations and within agency operating plans.

ICF/DDH experience is preferred.

QUALIFICATIONS :

High school diploma, A.A. Degree, or 60 plus units from a college or university, or equivalent experience in a lead staff, facility management or supervisory position in a residential facility preferably with intellectual disable adults.

Good communication skills (verbal and written), dedication, flexibility and teamwork participation.

Computer skills, General knowledge of Microsoft office software, Word, Outlook & Internet Explorer.

Must be capable of obtaining CPR and First Aid Certification and meet all ICF In-Service requirements within required timelines.

Must be capable of taking and obtaining Non-Violent Physical Crisis Prevention Intervention (CPI) card.

Must possess a valid California driver's license, and driving record that meet agency insurance requirements and Motor Vehicle Operating Standards.

A broad general knowledge of:

Developmental disabilities, including current issues and trends, Laws relating to people with developmental disabilities.

Teaching/training techniques.

Develops implements and maintains systems that establish a harmonious working environment that encourages employees to be creative, productive and truly vested in the goals of Peppermint Ridge affording compassion, integrity, practicality with clients and staff.

Must be willing to complete a health screening physical examination that includes drug test and a back examination; must have the unrestricted capacity to employ physical restraint techniques in a professionally safe manner.

Must be willing to complete a personal background investigation conducted by the State of California; and must meet Department of Health Services standards for employment.

Must be willing to complete a health screening physical examination that includes a drug and alcohol screen, TB screen, 50 lb. lift test and lumbar x-ray. Client Care and housekeeping requires lifting, pulling, tugging, twisting and bending at the knees.

Must be willing to work with culturally integrated workplace and be willing to respect human differences based upon race, age, handicap, religion, sexual orientation, political persuasion, place of origin, color, or any other condition that distinguishes people from one another.

DUTIES AND RESPONSIBILITIES:

Except for relief periods, Facility manager is responsible for the home on a 24 hour basis. This includes AM, PM and NOC shifts when necessary. Ensures the purchasing of grocery items, and other supplies as needed; menu planning and food preparation with the correct diets as ordered. Confers with the house Nurses to ensure the correct diet orders and adaptive eating equipment is updated and post for staff to refer to; alerts staff of any changes.

Implements and documents training programs described in the Individual Service Plan (ISP's). Supervises in maintaining a program of instruction in living skills integrated into the daily living routine, including:

Self-help and daily life skills

Independent living skills

Communication skills

Social/behavioral skills

Community integration skills

Working with the Interdisciplinary Team to:

Identify the extent and nature of each resident’s strengths and deficits.

Develop an activity/training schedule for client programming.

Ensure that behavior plans and ISP objectives are up to date and being followed correctly.

Ensures that resident health and personal hygiene is checked daily:

Confers with nurses, QIDP/A, Consultants, physician, dental hygienist or dentist when indicated.

Schedules monthly or necessary Doctors’ appointments along with follow up appointments, ensures that staff follow through with personal care instructions, new medications and treatment orders

Supervises and checks health and personal hygiene daily ensures staff properly document and report any problems to the Facility Manager, Nurse and QIDP/A.

Assists with personal care daily (dressing, bathing, assistance with toileting and eating needs). Dressing non-mobile clients requires two staff.

Passes and verifies medications and treatments as assigned on work schedules when needed.

When required; accompanies residents for professional medical/dental appointments and hair care.

Provides clients with their adaptive equipment in the facility and community outings (walker, wheelchair, gait belt, AFO’s supports). Ensures they are replaced and updated when ordered.

Ensures full compliance with budgetary plans and controls overtime. Ensures staff are arriving and properly clocking in on time to control overtime and stay in compliance with budgetary plans. (monitor’s time-correction sheets when staff are unable to clock in on computer)

Ensures Direct Training Staff read, sign and follow each client’s ISP objectives daily along with following the current procedures outlined in their behavior plans. Must ensure and adhere to the client’s activity schedule.

Works with staff to maintain high standards of cleanliness throughout the home through laundry, sweeping, moping, kitchen clean-up, vacuuming, taking out the trash and organizing client dressers/closets. Ridge vehicle cleanliness inside and out along with the grounds of the home.

Schedules employees working hours and calls substitutes as needed or covers the shift personally when a substitute is not available. Ensures that resident chores are carefully supervised.

Ensures that all operating and personnel reports are completed and submitted in a timely manner.

Serves as a liaison between assigned home and others. Keeps parents posted on residents status and progress. Works with QIDP/Administrator and caseworker in providing appropriate programs. Communicates with day program staff in person and in writing concerning resident’s health and education.

Properly orientates new staff including on-call staff to house procedures, duties and clients. Responsible for 3, 6 or 9 month evaluation on new staff when appropriate, and evaluates all employees annually to ensure employee accountability for resident care and in-service training.

Provides supervision to all house staff to ensure that all ICF in-service training requirements are met for all staff and follow up with staff in-service records to ensure that in-service requirements are maintained and hold staff accountable for deficiencies in training hours. Provides appropriate disciplinary actions to all staff when requirements are not met.

Assists in client record keeping, such as daily progress notes, medical charting and reviews all notes daily in client charts made by staff. Reviews client medical and administration charts when needed.

Ensure medications are administered properly and signed off on by both medication passer and verifier during each pass and logged as per licensing requirements.

Ensures all Doctors recommendations after visits are being followed and relayed to the house nurse. Ensures proper notification is posted in the home to make staff, FM and RN aware of any changes.

Reports employee on the job injuries/accidents, no matter how slight to QIDP and Human Resources immediately and ensures staff have properly filled out documentation that is to be submitted to HR in a timely manner.

PERIODIC RESPONSIBILITIES:

Arranges for transportation of residents as needed.

Responsible for coordinating leisure time activities both within the home and of campus including Angel Hall activities coordinated by our activities director. Purchasing age appropriate activities for residents as needed.

Notifies maintenance department of needed repairs of equipment and furnishings etc.

Ensures the physical and psychological needs of clients are being met; develops; and strives to maintain a high level of morale in a professional setting.

Serves on Admissions Committee for consideration of candidates for the residence. Attends monthly supervisor and house meetings along with quarterly HRC meetings. Ensures staff are attending monthly and annual training requirements and are held accountable when not met timely.

Keeps notes and turns in for bi-monthly newsletter articles to the Assistant Director.

Assumes responsibility for dispensing and maintaining accurate records of resident’s personal money and ensuring P & I procedures are met. Assumes responsibility for dispensing and maintaining accurate records of monthly shopping purchases with the use of cards, program money, house fund and/or clients P&I for spend downs

Delegates duties to staff as appropriate, retaining overall responsibility for successful completion.

Assures responsibility for clients personal shopping and haircuts when necessary. (checking the money out when needed and collecting the receipts inputted to client ledgers online)

Performs other duties as assigned.
