General Duties:

Supervises Personal Assistants

Performs resident assessments.

Assists the Cognitive Enrichment Director with creating service plans with appropriate goals, problems, and revisions based on resident needs.

Orders and administers medications and treatments as prescribed by the physician.

Awareness of prominent medication interactions and side effects; monitors residents for occurrences of such and notifies physician/responsible party. Adheres to infection control policies and procedures. Counts controlled substances with off-going associates assisting with medication; assumes responsibility for assuring accuracy of count before accepting keys. Secures and corrects reconciliation of controlled substances during a tour of duty.

Provides assistance with activities of daily living to Residents in accordance with personalized care service plan.

Education

Graduate of an approved L.V.N. program (per state requirements).

Qualifications

Current LVN license.

One year, minimum, experience in the health, human services and/or community service field.

Must possess interpersonal relations skills and capability of relating to a variety of people and personalities. Initiative, judgement and leadership skills also required.

Current First Aid and CPR Certification

Demonstrates competency in the performance of his/her duties through utilization of appropriate scientific principles and sound clinical judgement.

Must possess excellent oral and written communication skills to effectively communicate with seniors, family members and staff members.
