Discover why U.S. News & World Report has named us one of America’s Best HospitalsWhat you will be doing:The outpatient Clinical Oncology Social Worker is responsible for the planning, organization and implementation of social work services to cancer patients and their families. Develops patient care goals and treatment plans as agreed upon by the patient’s care team regarding psychosocial issues, and in agreement with the Cedars-Sinai Oncology Enterprise Patient and Family Support Program Social Work Care Model. Job Duties include:

Follow regulatory requirements (i.e. JACHO and BBSE), and standards and scope of practice as outlined by AOSW and NASW. Practice social work services according to applicable laws.

Manage and monitor distress screening tools. Use these to triage patient needs, create evidence-based treatment plans and refer them to the appropriate resources.

Respond in a timely manner to referrals initiated by various members of the medical oncology care and Patient and Family Support Program teams.

Assess the psychological, physical and environmental factors impacting cancer patients, and their families.

Provide behavioral health interventions to cancer patients and their families throughout the trajectory of the patient’s cancer illness.

Connect patients with appropriate resources including cancer specific grants, transportation services, home health options, community cancer wellness programs, insurance providers, and hospices.

Participate in and lead the proper reporting of suspected abuse/neglect and other government-related reporting such as Adult Protective Services, Office of the Public Guardian, Long-Term Care Ombudsman, Community Care Licensing, and law enforcement. Advocate for patient rights and self-determination in the medical system.

Discuss goals of care including: palliative care, hospice care, and advanced care planning with patients. Participate in documenting the patient’s wishes in their electronic medical records.

Evaluate caseload daily. Assess achievement of short- and long-term patient goals.

Provide follow-up and outcome communication to the referral source (oncologist, Patient and Family Support Program colleague, family, home health personnel, community social worker, health plan)

Collaborate with the transdisciplinary treatment team of physicians (psychiatry/cancer rehabilitation and survivorship/supportive care medicine), nurses, social workers, care managers, registered dietitians, chaplains, and medical staff to ensure integration of care.

Attend weekly inter/multidisciplinary team meetings. Participate in weekly group and individual supervision. Provide On-Call Social Work Services.

What We are Looking for:The position requires experience and facility in brief therapy interventions, counseling, crisis intervention, problem-solving and conflict resolution, patient and family management, transdisciplinary collaboration, bio-psychosocial assessments, navigation, education, advocacy, and community resource linkages.Requirements:Education: Master of Social Work requiredLicense: Requires maintaining active registration (ASW) in good standing with State of California. LCSW highly preferred. Hospice and Palliative Social Worker Certification desirable.
