Join us, and discover why we are #1 in California and nine years in a row on the "Best Hospitals" Honor Roll.As a onsite Pharmacy Program Coordinator - Clinical Diabetes Stewardship Pharmacist at Cedars-Sinai Medical Center, you will assist in leading diabetes care across Cedars-Sinai Medical Services and work with diabetes interdisciplinary team to deliver safe and effective services. Leads in developing a pharmacy stewardship model for diabetes care, reducing patient harm related to insulin administration, optimizes institutional glucometric outcomes, and enhances education efforts across all interprofessional service-lines within the organization. Assists in constructing a real-time surveillance tool embedded within the electronic health record (EHR) to allow for identification of patients with glycemic excursions, enhancement of care triage, and improve delivery of pharmacotherapeutic recommendations. Pharm.D. or equivalent degree from an accredited school of pharmacy.

Completion of a PGY1 and/or PGY2 residency or equivalent practice experience.

License/Certification/Registration Requirements:

Current pharmacist licensure in the State of California.

BLS certification as outlined in the Pharmacist Responsibilities in Cardiopulmonary Resuscitation (C.P.R.) and Code Blue Procedure: Pharmacy Policy and Procedure

What else are we looking for?

Three (3) years or equivalent experience as an inpatient pharmacist in a major medical center.

Possesses excellent clinical data base.

Possesses experience in Medication Use Evaluation.

Possesses excellent verbal and written communication skills.

Possesses software skills e.g., Microsoft Office.

Comprehensive knowledge of medical terminology as related to drug usage.

Comprehensive knowledge of common disease states and pathologies.
