Description:Providence is calling a Speech Language Pathologist (Part time 24 hours a week/Day shift) to Providence Little Company Of Mary Home Health. This position will be traveling to see patients in the Santa Monica/Hollywood area.$5K Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begins on your first day of employment.The Speech Therapist is a licensed professional who is responsible for maintaining standards of professional and ethical practice in accordance with regulatory agencies and the American Speech Therapy Association (ASHA). The Speech Therapist demonstrates proficient clinical knowledge and skills in providing and initiating treatment programs for individuals with swallowing, speech and cognitive impairments. Utilizing a collaborative team approach in providing care to; young, middle and older adult patients.In this position you will:Demonstrate proficiencies in supervising and administering the performance of duties and responsibilities related to patient care and protection of patient rights:

Perform patient evaluations and clinical proficiencies with use of therapeutic equipment and treatment principles within scope of licensure.

Provide family/ caregiver training when needed and involves the patient & family in goal setting and addressing the educational and discharge needs of the patient.

Reassessment are completed as stated in the standards of care.

Clinical documentation including discharge summaries is completed per departmental standards.

Communicate patient progress with interdisciplinary team and other disciplines.

Adhere to and supports all regulatory standards.

Information Management:

Documentation is completed within timeframe specified in departmental policy and procedure manual

Documentation is legible, organized and professional.

Demonstrate a working knowledge of the hospital information system and confidentiality policies

Document short and long term goals in specific measurable time frame.

Maintaining confidentiality of records.

Perform billing duties accurately and timely.

Leadership and Supervision:

Demonstrate the ability to communicate (verbal or written) in a positive, clear and professional manner. Offers constructive timely feedback in a confidential and non-threatening manner. Praises publicly, counsels confidentially.

Assist staff in completing tasks or duties, directly supervising their clinical and professional actions.

Assist with new and temporary staff orientation process.

Demonstrate good listening skills, respond to needs and request of others quickly and efficiently.

Professional Growth/Development:

Demonstrate responsibility for own professional growth and practice.

Attend all mandatory staff development activities.

Participate in community education / promotional activities.

Performance Improvement:

Meet all orientation and annual skill competencies required by organization and regulatory agencies.

Participate in organizational and departmental performance improvement projects.

Contribute to organizational and departmental performance improvement objectives.

Qualifications:Required qualifications for this position include:Sufficient education for license

Current California State Speech Therapy License

BLS certificate

Current CA drivers license and auto insurance

Good working knowledge of general and specialized clinical equipment

Ability to organize and prioritize work, completes assignments on time

Ability to establish and maintain a courteous and cooperative working relationship with patients, families and coworkers

Ability to type and use a computer efficiently

Ability to work effectively independently and with a team
