Evaluates patient's previous living circumstances in context of current medical status to develop an appropriate discharge plan. Minimizes patient anxiety concerning illness and facilitates post- hospital referrals. Interacts with and supplements the health care team in order to enhance the social, emotional and psychological care of the patient/family/significant others.Job RequirementsMinimum Education Requirement:MSW is required from an accredited school of Social Work.Minimum Experience Requirement:Minimum of 1 year experience working in a hospital environment is preferred. Excellent customer service skills required.Minimum License Requirement:May be required to have a current, valid CA driver's license to drive between sites.
