Personal Care Partner | QMAP (Part-Time Evenings)

Schedule: Friday, Saturday, Sunday | 2:00 p.m. – 10:00 p.m.Join a community where older adults thrive, we welcome their voices, and choices as this is their home. We’re seeking a QMAP who believes in empowered older adults and truly collaborative care.You will:Support residents with daily personal care and ADLs (bathing, dressing, grooming, meals, mobility).Build respectful, caring relationships with residents and families.Monitor safety and well-being and promptly report changes to nursing/supervisors.Assist with medications under licensed nurse supervision, following all “five rights” and documentation procedures.Communicate and collaborate with the care team to support resident goals and preferences.You are:Compassionate, patient, and respectful of resident choice.A strong team player and communicator.Experienced in caregiving and/or medication assistance (or willing to be trained, per state law).Able to work Fri–Sun, 2p–10p consistently.Be part of a mission-driven community that centers dignity, autonomy, and partnership with older adults.
