Job Description

  • Complete initial and recertification assessments for pediatricclients receiving IHSS.
  • Assist in scheduling visits according to the approved care plan assessment hours.
  • Administer a skills validation test for the Attendants.
  • Verify and document attendant skills and competency to perform IHSS and basic consumer safety procedures.
  • Counsel attendant staff on difficult cases and potentially dangerous situations.
  • Consult with the client, authorized representative, or attendant in the event a medical issue arises.
  • Verify that the attendant is following all services set forth on the IHSS care plan.
  • Review the care plan and the Physician Attestation form upon initial enrollment, following any change in condition, and upon the request of the client, the Authorized Representative, or the case manager.
  • Provide in-home supervision for the client as agreed upon by the client or their Authorized Representative.
  • Ability to assist with occasional after-hours calls related to client safety.
  • Ability to participate in care coordination with Medicaid case managers and home health care agencies to ensure IHSS Medicaid services continue without interruption.

Qualifications

  • Licensed Registered Nurse in the state of CO.
  • Pediatric experience preferred.
  • Excellent telephone, communication, and documentation skills.
  • Able to read and write legibly.
  • Proficiency in the Microsoft Office suite. Ability to operate within an electronic medical record.
  • Able to communicate effectively with fellow co-workers.
  • Able to recognize own limitations and request help when needed.
  • Must have a desire and willingness to learn and demonstrate the ability to accept supervision and follow direction.
  • Must be dependable, reliable, and responsible.
  • Must be able to work with others in a professional and ethical manner.
  • A valid Driver’s license and reliable transportation to complete in home visits

Additional Information

This is an exempt/salaried position with a salary range of $65,000-$75,000.
