Social Worker LCSW - Hospital Care Management (Job ID: 7870)Job SummaryUnder minimal supervision, plans, organizes and implements social worktreatment for clients and families. SICU social workerhas to provide crisisintervention, adjustment to illness, grief and loss, facilitate familymeetings, understand complex surgical issues which significantly impactand/or limit a patients functioning. This work includes assessing the needsof patients and families and carrying out interventions such as individual,group, and family therapy consistent with the spirit of evidence-basedpractices such as motivational interviewing and acceptance and commitmenttherapy. The work also includes identifying and connecting patients toresources to assist with their ongoing healthcare.ResponsibilitiesKnowledge, Skills & AbilitiesKnowledge, Skills and Abilities 1. Knowledge of crisis intervention andbrief and long term therapy processes. 2. Knowledge of patient populations'cultural, religious, ethnic, and social systems in interactions, careplanning and education. 3. Knowledge of medical terms, abbreviations,diagnosis, treatment and prognosis and demonstrates this knowledge verballyand in writing. 4. Skill in navigating and documenting within a medical recordsystem. 5. Skill in providing care appropriate to the age of the patientsserved within the unit. 6. Skill in establishing a treatment alliance andengaging the client system in goal setting and prioritizing. 7. Ability tomanage the professional relationship including termination. 8. Ability torespond to trauma or psychiatric emergency.EducationMaster's Degree in Social Work from a CSWE accredited college or universityrequired.Experience2. Two years of full time post Master's Degree social work practice underthe supervision of a licensed or equivalently qualified social worker or otherDORA approved professional practitioner required. 3.
