Physical Therapist - Per Visit or Salary

HealthPro Heritage at Home is seeking to add a full-time Physical Therapist - PT for home health in our Wallingford Branch. Coverage area will include Wolcott, Southington, Prospect, Cheshire, Meriden, Wallingford, Waterbury, and Naugatuck.

Flex schedule

Rate range from $65.00-$110.00 depending on visit type. Position is “pay per visit”

Salary positions are available depending on market/coverage area

Review available resident information and evaluate the impact of this information on the assessment and treatment process.

Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs.

Bachelor/Masters/Doctorate of Science degree in Physical Therapy.

Current license in the state of practice.
