Job Description for Individual Goods & Services Supervisor

The Individual Goods & Services Supervisor is responsible for monitoring, coordinating and supervising the direct services personnel who implement the daily services and activities deriving from a young man’s (JH) Individual Services Plan. With the supports provided by these direct services personnel, JH is moving into his own apartment after years of living in group homes and other congregate care living situations. The Individual Goods & Services Supervisor reports directly to the Employer of Record.

Requisite skills and training

An associate degree in a relevant field such as human services, psychology, developmental disabilities, etc.

At least one year either providing direct care services or supervising others who are providing such services for individuals with special needs

A verified history of leadership, reliability, excellent inter-personal skills and the ability to follow through

Knowledge and experience in community programming in natural environments

Expected Responsibilities

Will assist in finding opportunities for Jehan, socially, volunteer and work.

Will create a weekly calendar of scheduled events & activities, e.g. shopping, professional appointments, exercise, free time, recreation, home visits, special events, etc., as well as the staff schedules and place it in a conspicuous location where it will be easily viewed by JH and staff.

Will assist in the hiring of staff if necessary.

Will work with the Employer of Record as needed.

Will assist train all staff as per their job description.

In the event of an unanticipated staff illness or absence the supervisor will either identify another staff member willing to temporarily fill in or will fill in instead

Will follow all requirements of the electronic visit verification (EVV) including completion of EVV training.

Will assist all direct services personnel in using the EVV system for recording time worked and services/tasks completed.

Will actively participate in the ongoing monitoring of supports and services

Will help ensure that each direct service person documents the start and end time for the service provided to JH for each date worked.

Will prepare timesheets/EVV, receipts, mileage logs, and invoices on required forms.

Will review weekly payroll to verify that weekly services and supports took place and were provided by the direct services personnel

Will review the monthly expenditure reports provided by the fiscal intermediary

Will file, oversee and verify JH’s personal funds and entitlements ( redetermination, rent subsidy)

Will identify in ongoing in-service training or consultation needs

Will take on various responsibilities from time to time as requested by the employer of record
