RN Case Manager

Cant Find the Right RN Opportunity We are Looking to Hire A CT Licensed RN Care Manager!We are currently HIRING for Registered Nurses for Connecticut's leading Healthcare Network. The RN is responsible for admission of patients for home care services, development of the patient plan of care ,coordination with patient's provider and other disciplines for the provision of services, and safe delivery of patientcare.Job Duties:

Collaborates and speaks with providers/physicians to ensure patient care is adequate

Documents all relevant information about patient into emr system

Creates own schedule according to patients needs

Assess payment and responsible for ongoing authorizations

Educates/instructs patients on medical maintenance

Care/Discharge plans

Additional Skills & Qualifications:

Reliability

Epic experience

1+ years of nursing experience is required - home care & skilled nursing experience is preferred

EMR experience is required

Active State of CT RN License
