The role you’ll contribute:The Operations and Data outcomes nurse collaborates with clinical departments across the division to improve quality and outcomes to meet national guidelines and regulatory requirements for the service lines supported. Responsible for ensuring clinical outcomes accurately reflect the care provided at AdventHealth. Utilizes clinical expertise to review patient care episodes across the continuum and make appropriate inferences of clinical outcomes to support initiatives around evidence-based practice for the services lines supported. Serves as the clinical content expert on national guidelines and specialty-based outcomes. Responsible to collaborate with management and care coordinators to identify instances of clinical variability to provide opportunities for continuous performance improvement. This position is accountable for the ongoing monitoring of program quality and assists with program marketing needs. The nurse actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.The value you’ll bring to the team:

Completes longitudinal patient follow-up as assigned by interviewing and assessing patient health status across the continuum, to meet regulatory requirements. During patient assessments, must be comfortable navigating complex patient needs and referring to appropriate provider for further care. Maintains accurate databases to ensure follow-up deadlines are met according to specialty service line requirements.

Facilitates an evidence-based, multidisciplinary, multi-specialty approach throughout the care continuum. Assists with the monthly service line meetings presenting clinical data and recommending process improvements.

Coordinates quality and outcomes clinical projects across specialty service line(s) (as assigned) with hospital and physician leadership to improve quality and value, in alignment with the organization's strategic imperatives.

Responsible for seeking opportunities to maintain clinical expertise to incorporate latest developments and trends related to assigned national clinical guidelines. Attends educational conferences and training as appropriate.

The expertise and experiences you’ll need to succeed:Minimum qualifications :

Graduate of an Accredited School of Nursing

Current, valid Florida State of Board Nursing Registered Nursing License.

CPR Certified

Bachelor's degree in nursing or 5 years of specialized nursing experience

Preferred qualifications:

Three to five years of professional nursing experience

Bachelor’s Degree

Bilingual/Spanish
