Case Manager - RN

Description

The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care, and is discharged to the least restrictive environment.

Serving the community since 1976, Northside Hospital is a 288-bed facility, offering a full range of services including interventional stroke care, full-service orthopedic and spine treatment, a 24-hour emergency department, Chest Pain Center with PCI, preventative and corrective cardiac care, general surgery and a graduate medical education program.

As the first Comprehensive Stroke Center in St. Petersburg, physicians and staff enjoy providing patient care in the technologically advanced facility. With campus-based specialized units, such as the Tampa Bay Heart Institute, certified Afib & Heart Rhythm Center (SCPC) and the Neuroscience Institute for Interventional Neuroradiology and neurologic care, Northside Hospital continues to enhance and expand its services. Qualifications

Licensure/Education:

Current State of Florida RN licensure

Completion of an RN Diploma program or related healthcare program

National Case Management Certification strongly desire

Qualifications:

Minimum of three (3) years recent experience in an acute-care hospital setting required
