The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care, and is discharged to the least restrictive environment.

Qualifications

Licensure/Education:

• Current State of Florida RN licensure
• Completion of an RN Diploma program or related healthcare program
• National Case Management Certification strongly desire

Qualifications:

• Minimum of three (3) years recent experience in an acute-care hospital setting required
• Case Management experience is preferred
