Responsibilities

Provides clinical expertise in the nursing management of the patients, documentation, and care planning

Processes start of care workflow once start of care visit has been completed

Participates in IDG

Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary

Maintains orders in HCHB

Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed

Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned

Upholds effective communication with referral sources to ensure smooth transition of admissions

Qualifications

Associate degree in Nursing with current license

Three years practice as a Registered Nurse

Previous Hospice experience preferred

Knowledge of the Hospice Philosophy of Care

Ability to use Microsoft Word, Excel, Outlook, and other software programs
