Home Health Community Liason

Busy Home Health Agency is searching for a Community Liason. Responsibilites include:

- Identify, evaluate, and prioritize potential referral sources within the agency’s territory

- Develop and execute a comprehensive outreach plan, increase awareness and drive engagement with a variety of referral sources. The plan should include specific strategies and tactics to reach different referral sources with appropriate and tailored activities

- Establish and maintain strong business relationships with a variety of new and existing referral sources

- Research and participate in marketing and networking events, health and wellness fairs, community events to enhance visibility of agency services in the community and generate new relationships and business
