Child Welfare and Pediatric experience, or a background working with foster and/or adoptive children, are highly preferred.Position Purpose: Manages community and resource coordination activities to ensure members are being connected to local resources and care management and are being supported throughout care. Manages community and resource coordination team members and escalated issues related to members.

Manages and reviews all community and resource coordination activities including member outreach and ensuring members are connected to local resources throughout care

Oversees and approves community and resource coordination tasks to the team based on state and contractual requirements and member needs

Partners with community and resource coordination and care management team leaders to ensure effective, appropriate care of the member and addressing member needs

Ensures compliance with state and regulatory guidelines and standards

Sets goals and objectives for community and resource coordination team to achieve a more effective high-quality care across services

Works with care management senior management to provide updates and insights on community and resource support coordination team goals

Provides feedback to community and resource coordination team to improve community resources, outreach services and connecting members to community resources to provide necessary care in a cost-effective manner

Educates and provides guidance and/or resources for community and resource coordination team to address member and/or provider issues and concerns

Assists community and resource coordination and care management senior leadership with onboarding, hiring, and training community and resource coordination team members

Ensures adherence to service level, quality and KPI expectations

Performs other duties as assigned

Complies with all policies and standards

Education/Experience:Requires a Bachelor's degree and 5+ years of related experience.Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.Licenses/Certifications: Driver's license requiredPay Range: $72,400.00 - $130,100.00 per yearCentene offers a comprehensive benefits package including: competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
