Position Description

Come join the Sequent Team! Sequent Health Physician Partners is seeking qualified candidates as RN Care Managers in Gwinnett’s premier clinically integrated network (CIN). A Care Manager’s primary responsibility is to provide care management and coordination of clinical activities for patients with acute and chronic disease and to promote population management through effective education, self-management support, goal setting, and timely health care delivery.

What you will be working on…

  • Further Sequent’s care management programs aimed at preventing illness and improving individual well-being, clinical outcomes and quality of life for patients with chronic illness.
  • Engage patients using motivational interviewing techniques that determine patient’s health, social situation, physical environment, behavioral health, substance use, etc.
  • Develop and implement plan of care that address patient’s medical needs and barriers.
  • Coordinate and facilitate timely interventions to assist patient’s progress toward health goals, provide education, and evaluate the need for modification to the care plan.
  • Electronically document all activity in population health platform (PHM) and subsequently coordinate with provider(s) and care team to minimize fragmented care and foster appropriate utilization of services.
  • Develops and operates Chronic Condition Management (CCM) Program targeted to specific patient populations with chronic illnesses.
  • Coordinate continuity of care with patients and physicians following hospital admission, discharge, and ER visits

Position Requirements

Minimum Qualifications:

  • Current unrestricted license as a registered nurse (RN) in the State of Georgia OR an accredited advanced healthcare related degree
    • 3-5 years of clinical care experience

Preferred skills or experience…

  • Knowledge of the principles and practices of population health, care management, case management, utilization review and clinical quality improvement Experience providing clinical care in a primary care setting
  • Clinical Assessment, motivational interviewing and patient education skills
  • Analytical skills necessary to prepare reports.
  • Experienced user of clinical information systems (i.e. electronic health record, practice management systems, disease registries, clinical analytic systems)
  • Ability to work collaboratively with physicians and healthcare administrators
  • Ability to work independently with under general direction
  • Skill in oral and written communication to address inter- and intradepartmental concerns, solve problems, and address conflict.
  • Knowledge and experience in educating, supporting and promoting self-management of diabetes.
