Purpose of Position or Position Summary: Manages caseload of patients with psychiatric diagnoses and their families as part of an interdisciplinary team; contributes to overall program by facilitating individual, group, and family meetings; leads groups; makes managed care calls; and establishes patient/discharge planning. MANAGES CASELOAD OF ASSIGNED (UP TO 8) PATIENTS · Completes (biopsychosocial) assessment and treatment plan within 72 hours of admission · Meets with assigned patients to review and have patient sign treatment plan · Meets individually with assigned patients within 24 working hours of admission, and 3 times weekly thereafter · Obtains appropriate releases of information as indicated · Performs face-to-face evaluations as requested · Actively participates in treatment planning meetings · Collaborates with community service providers as indicated · Facilitates family/team meetings as indicated · Completes comprehensive discharge plan which includes, but is not limited to: treatment recommendations, medication follow-up with physician; appointment with outpatient care providers; and patient placement as indicated · Communicates with the interdisciplinary team daily regarding patient progress or treatment related issues ACTIVELY PARTICIPATES IN OVERALL UNIT PROGRAM · Models appropriate social interaction skills, including boundaries, communication, conflict resolution, etc. with patients, families, peers, and others throughout the organization · Facilitates group meetings as scheduled (starts and ends group on time) · Provides immediate crisis intervention services to patients as needed · Intervenes when patient is dangerous to self or others, utilizing least restrictive interventions, but may include seclusion and/or restraint under the direction of an RN · Attends Community Meeting as needed · Presents unit in-services as requested · Participates in program planning and development meetings · Assists in creating and maintaining a positive work environment · · Covers responsibilities of absent co-workers, including groups · Assists in creating and maintaining an environment which is safe, clean, and conducive to patient comfort · Reports all patient/environmental safety concerns to Charge RN · Adheres to universal precautions, chemical safety, and proper body mechanics DOCUMENTATION · Completes clinical progress notes a minimum of 3 times a week · Documents treatment plan reviews within weekly time frames · Documents all communication with collateral contacts · Documents significant events as they occur (family meetings; network meetings; allegations of abuse) · Documents managed care reviews · Completes discharge summary within 24 hours of discharge · Completes other documentation/correspondence as needed MAINTAINS PROFESSIONAL RELATIONSHIPS AT ALL TIMES · Adheres to professional code of ethics · Complies with staff conduct and confidentiality policies · Maintains state and federal confidentiality regulations · Adheres to state guidelines of mandated reporting of child, adult, and/or elder abuse (i.e., physical, mental, emotional, sexual abuse) GROWTH AND DEVELOPMENT · Obtains individual clinical supervision as needed · Follows chain of command to deal with issues (brings forth clinical issues to clinical supervisor; brings all other issues to Unit Manager) · Attends and participates in 50% of unit staff meetings; reads and signs minutes of meetings not attended · Completes mandated training and critical competencies annually · Maintain licensure requirements · Completes self-evaluation annually · Assists in the orientation of new staff OTHER DUTIES AS ASSIGNED · Floats as requested · Assists in answering unit phones/doors · Performs other duties as assigned/requested MARKETING · Provides lectures to community and referral sources re: services provided at St. Mary’s Health System, and other topics as needed/requested · Communicates professionally with referral sources and returns phone calls promptly · Sends information (i.e., discharge summaries) to referral source and/or outpatient care providers in a timely fashion
