Admissions / Intake Coordinator - Skilled Nursing

Job Description

• Your primary goal is to coordinate the central intake / admissions process and support census growth for 6 centers in the Chicagoland area.
• You willwork closely with our external marketing and nurse liaisons.
• Provide clients (prospective residents,their families and referral sources) with facility and program specific information.
• Evaluate and assess potential patients for clinical appropriateness and placements into one of our family of centers
• Help toensure proper admission documentation at the building level.
• Participate in the facility specific sales and marketing team events and plans as needed.

Qualifications

• Prior experience in an Admissions Coordinator position in a Skilled Nursing Facility or Rehab Center is a must.
• Associates degree or higher is preferred
• Solid computer skills, including Microsoft Office products andPoint Click Care (PCC)
• Working knowledge of federal & state regulations and reimbursement (Managed Care, Medicare, and HMOs)
• Ability to work remote (from home) should you wish.
