Call Schedule: one call rotating every other week with the Physician

Practice Detail: Unique position to work with the Medical Director for Neuroscience Research and Memory Disorder Clinic at Ascension Illinois. Clinical care includes generative brain disorders, dementia, including Alzheimer's disease.

Facility: Ascension Alexian Brothers Medical Center and Ascensnion St. Alexius Medical Center

Location: Elk Grove Village and Hoffman Estates, IL

outpatient position for Nurse Practitioner or Physician Assistant that has experience in Neurology or Geriatric Medicine.

two clinic locations; main site in Elk Grove Village


Evaluates and treats patients with appropriate medical diagnostic and treatment skills.

Recommends, participates in, and works to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.

Accepts patient referrals from other physicians.

Participates in academic programs (e.g. medical education and research) as requested.

Provides appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.

Participates in outpatient consultation for specialty, which may include on call schedules.


BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office.

Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office.

Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.


Master's degree of Nursing required.
