This care includes, but is not limited to:· Bathing· Oral hygiene· Skin care· Foot care· Back care· Hair care· Personal care· Changing and/or straightening an occupied or unoccupied bed· Light straightening of client area· Positioning· Lifting· Transferring· ROM's· Adhering to specified infection control measures· Emptying foley catheter bags· Emptying colostomy or urostomy bags· Administering Fleets, oil retention, or soap suds enema· Non-sterile dressing changes· Vital signs as appropriate· Provides care to meet client's individual needs in an appropriate and safe manner.· Implements changes in care based on the plan of care in collaboration with the primary nurse.· Provides ongoing basic care instructions to caregivers as determined by the primary nurse.· Evaluates and communicates education with the client and caregiver/family based on their changing needs as directed by the primary nurse.Ensures quality care management by providing appropriate documentation, communication, collaboration and other follow-up and by using time, supplies and resources productively.· Ensures quality and safe delivery of care.· Respects client confidentiality in all communication.· Communicates to the physician changes in the client's condition or status.· Obtains, interprets, clarifies and implements physician orders.· Communication and collaboration with the interdisciplinary team as client needs change.· Attends and participates in the weekly Interdisciplinary Team Meetings and monthly nurses meetings.· Participates in departmental task teams as appropriate.· Reports to the primary nurse and/or Clinical Coordinator or designee all problems encountered with client care.· Uses time, supplies and other resources productively to provide responsive and cost-effective client care.· Completes appropriate documentation according to MMCI Hospice polices/procedures and regulatory body's requirements:o C.N.A. Visit Recordo Narrative Documentationo Death Noteo MMCI Event Reporting Formo Submits legible clinical documentation in a timely manner using the MMCI Approved Abbreviation List and Prohibited Abbreviations.Demonstrates punctuality and flexibility in the work schedule to meet the needs of the client and caregiver/family.· Recognizes and prioritizes client needs to ensure quality care.· Organizes visit schedule to ensure clients are seen within the scheduled workday.· Utilizes time so that client care is maximized.· Demonstrates flexibility in the work schedule to meet the needs of the client and caregiver/family.· Report all client schedule changes immediately to the Clinical Coordinator so that appropriate action may be taken.Demonstrates evidence of professional growth and development by maintaining a current knowledge base regarding nursing and palliative care trends. Shows evidence of quality improvement in the processes of hospice care.

Assumes responsibility for being informed about developments in health care and specifically Hospice care.

Identifies personal and professional learning needs by participating in educational opportunities.

Participates in the orientation of new C.N.A.’s as a preceptor when designated.

Maintains all departmental and educational requirements

Basic UPH Performance Criteria · Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.· Demonstrates ability to meet business needs of department with regular, reliable attendance.· Employee maintains current licenses and/or certifications required for the position.· Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.· Completes all annual education and competency requirements within the calendar year.· Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.Minimum RequirementsIdentify items that are minimally required to perform the essential functions of this position.Preferred or SpecializedNot required to perform the essential functions of the position.Education:High School diploma preferred.Experience:1-3 years of experience as a Nursing Assistant required.License(s)/Certification(s):Certificate of completion from an accredited nurses aide training program and successful completion of the IDPH approved Nursing Assistant Competency Examination.Must possess and maintain current Basic Life Support (BLS) certification within 30 days of hire/start date.Must possess and maintain a valid Driver’s License in the State of Illinois.Must possess and maintain auto insurance.Motor Vehicle Registration (MVR) must be run annually.Knowledge/Skills/Abilities:Required English Skills-Advanced reading skills-Advanced writing skills-Advanced oral skillsCommunication Skills-Ability to respond appropriately to customer/co-worker-Interaction with a wide variety of people-Maintain confidential information-Ability to communicate only the facts to recipients or to decline to reveal information-Ability to project a professional, friendly, helpful demeanorComputer Skills-Basic computer knowledge: Uses word processing, spreadsheet, e-mail application, and web browser. Comfortable within a Windows OS and learning new applications.-Familiarity with home care software and email useOther:Use of usual and customary equipment used to perform essential functions of the position.
