We Are HiringWe are hiring an LPN/LVN Licensed Practical Nurse or Licensed Vocational Nurse to work on the Home Health Team that is part of the Alexian Brothers Medical Center in Elk Grove Village, IL . Schedule: Full-Time, Monday-Friday, 8:00AM – 5:00PM, with weekend and holiday rotations.

Travel required in the territory service area of South Team region

What You Will DoProvide care to patients under the direction of a registered nurse or physician, functioning within scope of license.Deliver care to assigned clients and performs direct care in accordance with established plan of care.

Deliver safe care at all times that is in line with physician orders, home health policy and established nursing care standards.

Maintain communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation.

Report patient-related observations to supervising RN on a daily basis.

Administer medications and observe patients for adverse reactions to medications or treatments.

Provide documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirement and assures reimbursement.

What You Will NeedLicensure / Certification / Registration:BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required.

Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.

Practical/Vocational Nurse credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education:High school diploma or GED required.
