DescriptionJOB DESCRIPTION:

Coordinates care of sepsis patients from ED triage to discharge in 1-2 Emory Healthcare Hospitals.

Provides knowledge and skill in the care of Sepsis patients as a resource to staff, the interdisciplinary team and community caring for the patient within their specific area.

Maintains QA data with the goals of achieving clinical excellence, including obtaining and maintaining current knowledge of Centers of Medicare and Medicaid Services (CMS) SEP-1, 3-hour and 6-hour bundle measures.

Reviews each case of sepsis at respective hospital(s) to evaluate compliance with CMS SEP-1 measure 3-hour and 6-hour bundles.

Providing timely feedback to healthcare providers regarding SEP-1 3-hour and 6-hour Sepsis bundle compliance.

Responsibilities will be divided among sepsis coordinators to balance patient load.

Monitor the use and utility of sepsis tools within the assigned hospital(s), including the use and utility of sepsis alerts or predictive models, best practices advisories, and sepsis or related order sets.

Review both quantitative data and qualitative feedback from front-line staff.

Coordinate with hospital rapid response team to promote prompt diagnosis and care of admitted patients with sepsis.

Provides education and training in accordance to data results and trends identified along with feedback received from front-line healthcare providers, clinical documentation specialists, and quality management specialists on the utility of sepsis diagnostic tools and related order sets.

Coordinate with Emergency Department Triage personnel to promote rapid identification and rapid early management of patients with sepsis.

Coordinate with hospital bed placement coordination personnel to promote appropriate bed placement and resources for patients with sepsis.

Coordinate with Emory Digital and Office of Quality to ensure sepsis dashboards and other data products are available to inform sepsis coordinator priorities and clinical practice.

Identify and promote best practices around discharge coordination to reduce readmissions among patients with sepsis.

Assist in development of policies and procedures, order sets, and clinical pathways to improve quality of care, SEP-1 compliance, and sepsis program development.

MINIMUM QUALIFICATIONS:

Bachelors degree from an accredited nursing school.

Four years experience as a Registered Nurse, with one in the area of concentration.

Must have a valid, active unencumbered Nursing license or temporary permit approved.
