We Are HiringAMITA Health Home Care is hiring a PART TIME DAY SHIFT Registered Nurse to join our multidisciplinary home health care team. This position will provide you autonomy as you will manage your daily visit schedule independently. Schedule: Part -time days, Monday -Friday with alternating weekends and call, to equal 20 hours each week Service Areas: North West Suburbs You will receive a laptop or android tablet and iPhone, and your supplies will be delivered directly to the patient with no office pick-up required. This position will offer you support through our well-constructed onboarding and orientation program. What You Will DoProvide direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.

Serve as a primary coordinator of all disciplines for well-coordinated patient care.

Note and carry out physician and nursing orders.

Assess and coordinate patient's discharge planning needs with members of the healthcare team.

What You Will NeedLicensure / Certification / Registration:BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required.

Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.

Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:Diploma from an accredited school/college of nursing required.
