DescriptionShift 9am-5:30pmJOB DESCRIPTION: Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating, providing a link to research, serving as the primary point of contact for patients and families and providing indirect patient care. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.This employee will be working mainly at EUH and EUHM to identify patients from these ED that can be transferred to EDH. The duties of this position include but not limited to the following:Screen all patients in the assigned ED (EUH for the pilot) for transfer based on inclusion and exclusion criteria of the target hospital (mainly EDH as a target); talk frequently with ED providers to proactively review patients prior to admission time; for patients that screen as a good transfer candidate, personally have the conversation to get consent for the transfer using approved scripting as a guide; once patient agreeable, expedite connecting the transfer service and providers for transfer; help communicate necessary updates; identify and solve any problems with the transfers in real time; communicate with Transfer Service leadership and Hospital bed teams to help remove barriers in real time; follow up with any barriers to constantly improve processes.

MINIMUM QUALIFICATIONS: Graduate of an accredited school of nursing; BSN required. Two years nursing experience required, 1 year case management preferred. BLS required.PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required.
