Oncology social work services are available to patients and families throughout the cancer continuum, including prevention, diagnosis, survivorship, terminal care, and bereavement. The oncology social worker is an integral part of the health care team and contributes to the development and coordination of the overall treatment plan working in collaboration with other disciplines, linking patients with a variety of services necessary to meet the person’s multiple needs.Essential Functions/Responsibilities:% of Time(annually)Psychosocial Assessment· Use of high risk screening criteria to identify survivors and families in need of Social Work services· Completion of a psychosocial assessment to determine survivor and family’s strengths and needs relative to coping effectively with cancer diagnosis, treatment and follow-up cares· Knowledge about cancer and its treatment including level of understanding, reactions and expectations· Characteristics of the patient and or family’s social and economic, ethnic, spiritual and cultural concerns· Knowledge of financial/community resources to support patient and family40%Clinical Intervention· Uses a range of therapeutic and other interventions , including supportive counseling, group work, and education to address issues in each phase of the illness· Fosters coping and adaptation to cancer and its effects in order to help cancer survivors maintain or improve quality of life· Mobilizes new or existing family, system, and community resources to provide social and emotional support to cancer survivors· Develops a multidisciplinary care plan with survivor and family input based on mutual goals· Participates in weekly cancer conference and other care rounds integral to patient care 40%Involved in educational and support services· Participates in educational activities directed at community education and support· Collaborates with other health care providers in developing, implementing and evaluating educational materials for patients and their families; provides appropriate education for the client, the family, health care team and community support systems regarding cancer care including pre-op and post-op care· Facilitates relationships between physicians, vendors, departments and the oncology program· Participates in professional organizations related to oncology10%Basic UPH Performance Criteria · Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.· Demonstrates ability to meet business needs of department with regular, reliable attendance.· Employee maintains current licenses and/or certifications required for the position.· Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA, COC and other federal/state regulatory agencies or accrediting bodies guiding healthcare.· Completes all annual education and competency requirements within the calendar year.· Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.
