This position provides social work services via the Novartis/E3 (Engage, Empower, Evaluate) grant. The focus is providing targeted clinical social work services via the AIMS intervention to individuals with uncontrolled hypertension and other chronic health conditions, as well as unmet complex social needs. This position supports relevant programming for the aforementioned population and may serve as a representative of their department in meetings and presentations. This position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.Responsibilities:
Works directly with patients of all ages and their families to address unmet needs and provide the necessary social work services them achieve their care goals.
Carries out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
Provides care management and/or support programming for aforementioned population as outlined by Manager.
Documents all client interactions in either the Epic, LCM software and/or other spaces as indicated by Manager.
Maintains relationships with referring partners, both internal and external, through consistent and timely communication.
Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.
Represents the department at meetings, conferences or other public engagements on an as-needed basis.
Supports the Novartis/E3 larger interdisciplinary team, including clinical support for the Community Health Workers escalating cases.
Assist with providing trainings related to social work services and social care integration.
Engages in collaborative relationships with Rush and community-based providers in order to enhance patient care and meet programmatic goals.
Provides co-location to clinic as needed in order to collaborate with the team and serve patient needs.
Actively participates in clinical supervision.
Other information:Required Job Qualifications: Master’s Degree in Social Work. Licensed Social Worker (LSW) in State of Illinois.Preferred Job Qualifications: Two years’ experience in healthcare, care management, mental health and/or social services. Experience with relevant population(s). Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability. Knowledge of care management, community-based interventions, assessment, counseling and/or motivational interviewing. Computer literate (Microsoft Office and others).
