ResponsibilitiesPlans, organizes, develops and directs the social service programs to ensure the medical-related emotional and social needs of the residents are met and maintained on an individual basis. Supervises other social workers within the community. Travels within the division as assigned to strengthen social services processes until a new Social Services Director is hired The Social Services Specialist will continue to support the community and the Social Services function.

Assesses and identifies preliminary and comprehensive medical-related social and emotional needs of the resident. Conducts analysis on behavior expressions, provides feedback and recommendations to the interdisciplinary team.

Develops and implements social care plans to include identifying resident problems, needs and goals with the involvement of the resident, the family, physicians and other interdisciplinary team members.

Consults with physicians and healthcare team members in dealing with psychosocial factors relating to residents' illnesses, treatments, and discharge plans.

Documents informative and descriptive progress notes of services provided and resident responses to services provided. Provides oral and or written reports of the social services programs and activities as required.

Plans resident discharges including referrals to the community and family social services.

Provides information to residents and their families concerning available support agencies. Arranges for counseling and financial assistance programs to meet the needs of the residents.

Completes and audits residents advance care plans and advance directives.

Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; completing performance reviews; rewarding and disciplining employees; addressing complaints and resolving problems.

Assists or arranges for a resident’s communication needs by accessing resources such as translators, communication boards or other tools.

Identifies and promotes individualized, non-pharmacological approaches to care that meet the mental and psychosocial needs of the resident.

Assists in making appointments and arranging transportation for the resident/family as needed.

Develops, implements, and maintains an ongoing quality assurance program for the social services department.

Reviews, documents and assists in investigations and resolving complaints and grievances.

Possesses knowledge of federal and state regulations, including Medicare/Medicaid and Managed Care to arrange for needed services not covered by state regulations.

Assists with pre-admission and admission process.

Operates in accordance with current federal, state and local standards, guidelines and regulations, and established company policies and procedures.

Develops and maintains community liaison activities.

Reviews community outcomes, analyzes data, and provides written reports with recommendations.
