Apply Today!Job Summary and QualificationsThe ED Discharge Care Coordinator provides referral coordination post-discharge in accordance with established policies and procedures of MidAmerica Division and participating hospitals. In collaboration with the hospital’s medical staff, patients, patient’s family and other members of the interdisciplinary health care team, the ED Discharge Care Coordinator is a central point of contact for coordinating and facilitating the referral process. The Coordinator will also collaborate with physician offices receiving referrals to ensure continuity of care.What you will do in this role:

Facilitates ED discharge referral appointment scheduling by collaborating with all members of the interdisciplinary team including ED physicians, ED staff, physician office staff, hospital team members and CRM/Contact Center.

Documents referrals and subsequent patient interaction/referral outcomes for compliance, tracking and reporting.

Demonstrates skill in interactions with patients/family during the referral process.

Collects and provides data consistent with established processes and quality standards.

Identifies and resolves process and/or clinical issues related to the referral process.

Coordinator does not provide clinical care and does not extend or substitute for the more specialized services of a doctor, nurse or social worker.

Responds efficiently to multiple requests / functions simultaneously.

Uses resources effectively to reduce duplication of effort and promote efficient use and reliability of data.

Ensures all referral processes, procedures and communications are compliant and adhere to regulatory standards.

Adheres to the policy for handling of confidential information, files and PHI.

Contributes to a safe environment of care for the patients served.

Supports the mission, vision, and values of the organization.

Other duties as assigned.

What qualifications you will need:

Availability to work Monday-Friday, 10:30am-7:00pm

High school diploma

Minimum two years of customer service experience or patient experience

Must possess the interpersonal skills necessary to be an effective liaison among the medical/nursing staff, as well as the community.

Must possess the leadership and communication skills to develop and coordinate an interdisciplinary program.

Must be able to monitor and/or perform data collection; utilize data to develop processes addressing opportunities for improvement, and serve as team leader for implementation of process changes.

Leadership and communication skills to coordinate care

Ability to effectively educate families and peers.

Basic to advanced computer skills, Microsoft Word and Excel
