Our goal is to ensure all services consistently exceed quality measures and the expectations of patients and referring physicians.ResponsibilitiesThe Occupational Therapist develops and implements the home care plan for the occupational rehabilitation of patients under the direction of the attending physician and the appropriate clinical leader.

Performs initial occupational therapy patient evaluation.

Establishes and performs occupational therapy treatments according to the physician’s plan of treatment and in compliance with state and federal guidelines.

Provides patient care education to patients and other caregivers.

Participates in case conferences, summary reports, quarterly record review and annual program review, in-services and special case orientations.

Maintains productivity standards and availability requirements.

Qualifications

Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association.

Active registered OT license in the state of Kentucky

One year of OT experience required

Passion for excellence in quality of care
