Admissions RN

Job DescriptionInsight Global is looking to hire an Admissions RN for one of the largest home health and hospice organizations across the country. This Admissions RN will work a 7-on, 7-off schedule at the local hospice agency. The ideal candidate will have at least 2 years of clinical nursing experience and experience within a home health or hospice setting as a plus.This person will:

Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs.

Complete assigned admissions and patient care orders in an effective and timely manner.

Communicate effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibility.

Communicate effectively with operational, clinical and sales leadership regarding status of pending referrals.
