Collaborates with the treating psychiatrist/prescriber and interdisciplinary treatment team. Provide clients and families with education regarding mental illness, and/or addiction and discharge planning assistance for transition back into independent or supportive community living. Identify life areas of need and make appropriate referrals to ImCal programs and/or community agencies that best meet those needs. Keep informed and updated on community resources and make appropriate referrals to meet specific client needs.Provide crisis intervention/assessment to clients who are suicidal, SPMI, co-occurring, addicted, homicidal, gravely disabled, or otherwise high risk. This involves assessment for hospitalization, collaboration with the physician, and the family, and linkage with community supports, i.e., inpatient treatment facilities, police, social services.Communicate with various inpatient treatment facilities, court systems, DCFS, ImCal attorney, mental health advocate, clients, and family members to ensure continuity of care for clients.Attend clinical staff meetings and individual supervision as needed. Attend in-house and outside training as required. Provide presentations/in-service training to staff, as requested.
