We are currently hiring a Clinical Coordinator, HH HSPC to support our Hospice team.This is a full-time, dayshift position. Responsibilities will include quality, education, and some administrative on-call.As the Clinical Coordinator, you will be accountable for proactively supervising, guiding, and monitoring activities of the multidisciplinary team to affect quality patient care through accurate documentation, ongoing education, and coordination of the team's operation. Provides direction and oversight to ensure safe levels of staffing while maintaining efficient patient throughput. Serves as a resource for all staff and is able to apply competent knowledge and skills to achieve quality outcomes.Responsibilities:

Identifies opportunities for clinical quality as well as workplace improvement.

Applies evidence based practice as a regular aspect of professional practice

Builds and mends relationships among colleagues within the framework of a healthy workplace.

Role models keen cultural awareness including preventing and addressing horizontal violence and impairment.

Adheres to reporting elements and requirements set forth by all regulatory agencies for patient safety.

BSN preferred.

Minimum of 2 years healthcare experience- required.

Minimum of 1-year home health (or hospice) experience- preferred.

Minimum of 1 years progressive leadership experience with relevant clinical experience preferred. Licensure, Certifications, and Clearances:

Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located required.

Current Certification in basic CPR required.

Current driver's license in state of residence required.

Current auto insurance in state of residence required.

Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.UPMC is an Equal Opportunity Employer/Disability/Veteran
