The Community Health Navigator will also assist patients in access to a variety of medical and social services they need to maintain their health, with special attention to preventive screenings. The Community Health Navigator with collaborate with clinical teams to promote health and wellness and contribute to the goals and objectives of the patient's care plan.Job Description:Essential Responsibilities:

Manage assigned caseload of patients. Work cooperatively with other clinical personnel assigned to the same patient.

Provide consistent communication to the Care Management staff to evaluate patient/family status, ensuring that information and reports provided clearly describe progress.

Assist patients with completing applications and registration forms, as part of identified needs via a Social Determinants of Health screening. Conduct intake interviews with patients, including enrolling patients into, and/or referring them to, various health, assistance, and social service programs.

Help patients achieve personal goals based on shared decision making with their care team, and attend screening appointments.Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families.

Be knowledgeable about community resources appropriate to needs of patients/families. Provide referrals for services to community agencies as appropriate.

Help patients connect with transportation resources and give appointment reminders in special circumstances.

Follow-up with patients, such as by phone, or electronic platforms, including, but not limited to closing the loop on provider ordered preventive screenings. Work closely with medical provider's help to ensure that patients have comprehensive and coordinated care. Follow- up with patient should be continuous from initial identification through closure.

Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.

Attend regular staff meetings, trainings and other meetings as requested.

Required Qualifications:

High School diploma or GED required. Vocational or Technical training in Community Health Care Worker preferred.

0-1 years related work experience required in Experience working in a community-based setting for at least 1-2 years preferred

Ability and willingness to provide emotional support, encouragement, and motivation to patients.

Basic familiarity with computers. Ability to navigate at a basic level within web-based applications.

Preferred Qualifications:

Written and oral fluency in Spanish.

Experience working in a multi-cultural setting.

Knowledge of medical terminology preferred.

Problem Solving: Ability to address problems that are varied, requiring analysis or interpretation of the situation using direct observation, knowledge and skills based on general precedents.

Independence of Action: Ability to follow general instructions and procedures as provided. Work is monitored by supervisor/manager.

Written Communications: Ability to read, and write in English in order to understand basic safety instructions and take direction from supervisors; communicate effectively with patients, families and other medical center staff; and respond to basic questions.

Oral Communications: Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.

Knowledge: Ability to demonstrate basic knowledge of fundamental concepts, practices and procedures with the ability to use them in routine situations. Specialized knowledge not required.

Team Work: Ability to interact respectfully with other employees, professional staff and/or external contacts to offer ideas, identify issues, obtain information or deliver services.

Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.

Physical Nature of the Job:Sedentary work: Exerting up to 10 pounds of force occasionally in carrying, lifting, pushing, pulling objects.
