The Program Director(s) will report to the Leader, Population Health Programs, BILHPN and be responsible for design, development, implementation, evaluation and continuous improvement of a subset of BILHPN performance related programs. These may include but are not limited to quality, clinical documentation improvement, care management, pharmacy, care navigation and patient experience programs. The Program Leader will have accountability to ensure program goals and targets are met. He/she will collaborate with BILHPN leadership to provide strategic input into current and future program direction for specified areas of expertise. The Program Leader(s) will partner closely with Operations/Performance Management to ensure successful implementation and execution of the programs. The Program Leader will be accountable for program results in specified domainsJob Description:Essential Duties & Responsibilities including but not limited to:Oversight of performance related to specified population health programs.Collaborate with BILHPN leadership to strategically assess current and future specified programmatic needs to supportBILHPN goals.Maintain subject matter expertise in relevant program domains as well as understanding of nationaltrends/approaches/literature in the related space.Ensure that continuous monitoring results in program changes as needed to achieve goals.Ensure program development and implementation address the following domains to ensure success: Patient activation,Closing gaps in care, performance monitoring, training and education, strategy, collaboration with key stakeholders.Collaborate with BILHPN clinical leaders in program development.Maintain strong relationships with key BILH/BILHPN stakeholders and understand concerns/challenges when developingand implementing programs. Influence stakeholders and leaders to embrace programs and help deliver outcomes.Work closely with BILHPN Operations/Performance Management to facilitate program implementation.Ensure relevant BILHPN local care unit (LCU) support teams are connected across specific roles and have appropriateknowledge and skills to execute the specific program/initiative.Leverage data to understand performance, opportunities and support program goals.Collaborate with data analytics team to develop optimal reporting, scorecards and visuals to support programperformance.Collaborate with Epic Team to understand and develop point of care tools that may enhance LCU support teams’ and/orpractice care teams’ ability to be successful in relevant BILHPN programs.Develop and support a culture of collaboration, teamwork, accountability and continuous improvement.Have up to date awareness and understanding of latest technology/vendors available to support program performance.Have up to date understanding and knowledge of specific BILHPN contractual goals with both public and private payersas relates to specified programs.Organizational Requirements:Maintain strict adherence to all BILH and BILHPN Policies. Maintain courteous and effective interactions with colleagues and providers and stake holders. Demonstrate an understanding of the job description, performance expectations, and competency assessment. Demonstrate a commitment toward meeting and exceeding the needs of our customers and consistently adheres to customer service standards. Participate in departmental and/or interdepartmental quality improvement activities. Participate in and successfully completes Mandatory Education. Perform all other duties as needed or directed to meet the needs of the department.Minimum Qualifications:Education: · Bachelor’s degree required. · Advanced health care related degree preferred.Licensure, Certification & Registration:Experience: · Leadership experience in the pop health/value based care environment preferred. · Minimum 3 years experience in at least one of the following spaces required: care management, quality, pharmacy, care navigation, patient experience. · Experience with use of clinical and claims data to support pop health programs required.Skills, Knowledge & Abilities: · Strong leadership and management skills. · Collaborative and team player. · Knowledge and understanding of value based care, risk contracts. · Understanding of process improvement tools and techniques. · Facility with Microsoft Word, Excel, Power Point. · Experience with Epic, Epic Healthy Planet and Arcadia preferred.As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
