MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.Registered Nurse RNWe are offering a $5,000 sign-on bonus for full time hires! Mentor Adult Day Health is seeking a Registered Nurse to provide services, training, and support to address the medical, behavioral, and psychiatric needs of individuals. This is a per diem position.Responsibilities may include but are not limited to: Conduct a complete assessment of each participant

Develop individualized care plans

Oversee the management of high-risk medical conditions

Maintain contact with primary care physicians and specialists

Participate in quality improvement and educational programs

Train support staff in daily functions within their roles

Qualifications:

Current RN license

Graduate of an accredited nursing program

Two years of experience working with similar clientele

Bilingual - Spanish speaking preferred
